Navigating Cancer Announces Oncology Care Model Solution to Maximize Performance Payments for Participants

New Patient Relationship Management (PRM) platform proven to reduce costs and improve patient satisfaction

Navigating Cancer, the pioneer in patient-engagement technology for cancer care, is pleased to introduce a new solution for Oncology Care Model (OCM) participants to maximize their potential for achieving performance-based payments. The solution is powered by Navigating Care, the first and most comprehensive oncology patient care technology platform, used by thousands of cancer care providers to deliver cost-effective, patient-centered care.

Navigating Cancer’s OCM solution directly tackles the looming challenge facing OCM participants: reducing costs enough to qualify for performance payments. Despite the growing number of Medicare ACOs in the country, less than one third of all CMS ACOs in 2015 succeeded in reducing costs far enough below their target benchmark to qualify. Though the complexity of cancer care presents several cost saving opportunities, one disastrous hospitalization could completely negate the impact of more modest savings. Navigating Cancer’s innovative new platform promises to help OCM participants prevent such hospitalizations by pinpointing those patients most at risk and providing interventions in a highly coordinated and timely manner.

Navigating Cancer’s OCM solution draws upon various programs and tools within the Navigating Care platform. Navigating Care can be paired with a mobile application, Health Tracker, for remote monitoring of patient symptoms and side effects. Patients can communicate their outcomes back to their care team, where the collected patient information is surfaced onto a risk-stratified Triage dashboard so staff can monitor or intervene as necessary. At the same time, the platform is designed so that all members of the care team — navigators, care coordinators, social workers, and triage nurses — operate from a single and comprehensive view of each patient. The result of this patient-centered approach to care is more informed decision-making, better patient satisfaction and improved patient outcomes.

“Understanding the highest risk patients and how they are feeling outside the practice will be vital to knowing when to provide care to reduce costs,” says Gena Cook, CEO of Navigating Cancer. “Our remote monitoring platform enables faster care for patients where they feel more cared for and allows practices to care for their highest risk patients in an easy manner to avoid hospitalizations and readmits.”

Navigating Cancer’s OCM solution helps practices effectively manage patient incidents and risk. The platform allows care team members to work together to deliver timely interventions, as well as the proactive patient outreach necessary to stop acute patient incidents before they begin. Providers can track and report on all patient-focused MEOS requirements and take a more proactive approach to care. Simply augmenting the reporting features available in an EHR alone cannot replicate the efforts of a comprehensive care strategy. Participants using Navigating Cancer’s OCM solution will be well prepared to maximize on their performance payments come January 1st, 2017.
