Lalan Wilfong, MD, Executive Vice President, Value-Based Care and Quality Programs at Texas Oncology, recently presented a webinar entitled “Navigating Cancer: Improving Patient Outcomes: An Integrated and Intelligent PRM Platform” at the Association of Community Cancer Centers (ACCC) 47th Annual Meeting & Cancer Center Business Summit.
In Part 1 of this 2-part blog series, we showed you how Texas Oncology integrated Navigating Cancer’s Care Management platform into their practice, which streamlined clinic workflows and enabled quicker response times to patients. (Read part 1 of the blog here.) In Part 2, you’ll see how supporting patients with Navigating Cancer’s Health Tracker app continued to improve response times and enable better care by proactively involving patients throughout their treatment and increasing patient satisfaction.
Navigating Cancer’s Health Tracker remote monitoring system moves from reactive to proactive care
When patients use Health Tracker to report their symptoms, it reduces the number of phone calls to the clinic. Through the app, the patient’s electronic patient-reported outcomes (ePROs) go directly to the care team, streamlining the communication for quicker action.
Between July and December 2020, Texas Oncology
enrolled 4,373 patients in Health Tracker
73% of patients enrolled
completed at least 1 ePRO assessment
Patients maintained a
65% ePRO compliance rate
among all enrollees
Building on their success
After realizing such promising results from using Navigating Cancer’s Care Management and Health Tracker solutions, Texas Oncology is looking to further improvements in the future. Their goals are:
- Improving the patient experience of care (including quality and satisfaction)
- Improving the health of populations
- Reducing the cost of health care
Navigating Cancer is proud to partner with Texas Oncology and we look forward to helping them achieve their vision.
